If you are running on Windows, ActivePDF (www.activepdf.com) works pretty well and will do what you want it to do.

One from www.pdflib.com and one from www.fastio.com. The PDFLib is supposed to be better supportive of PHP. You can create PDF, but I'm not sure how much editing of one you are going to be able to do, perhaps you can. And you can definitely use a form to get data from to create a PDF.
