Rebase after refactoring of nbtree insertion scankeys. But so far it's purely mechanical, just to make it work - I guess I'll need to try to rewrite some parts of the patch, that don't look natural now, accordingly. And maybe to leverage dynamic prefix truncation per Peter suggestion.
