When I first bring a scan in (at 4000dpi), I sharpen it somewhere in
this range--
amount: 80% to 120%
radius: 0.7 to 1.2
threshold: always 0
I usually sharpen in baby steps, by setting the amount and radius on the
low end and then sharpening, and sharpening again, and again, going back
and forth using undo to decide when I've sharpened "enough". I usually
zoom the print to 'print size' and then move in another click (to a 25%,
50% or 100% magnification, not the in-betweens) when deciding on the sharpness.
When sizing down an already sharpened image for the web, I tend to use this--
amount: 20% to 50%
radius: 0.3 to 0.8
threshold: always 0
...and in this case, I view the image at 100%.
Hope that helps!

p.s. I use unsharp mask over sharpen because there's a whole lot more
control involved.
