I was wrong to think of Apple TV as a replacement for a cable box, but maybe
I was wondering if in some way, maybe your remote control might be the cable
box, but could be wrong, of course.

include audio as well as video. It has an HDMI and an optical audio

Basically its a small box which connects to a computer network and a TV,
the box allows you to stream content over your network and output that
to your TV and as a logical extension to that you can stream Internet
content such as Youtube video, netflicks content etc.

Another way round it, just hook the Apple TV to a TV using HDMI, new
TV'S with HDMI connectivity are bargains to be had.
