If you store lots of attachments you can also save them outside of the database if you wish. And you should not necessarily start with that, you can do that later on as well. I've seen one customer with 500,000 tickets and 18 GB database size; that's 36kB per ticket on average. And I've seen one customer with 1,500 tickets and 5GB database size; that's > 3MB per ticket on average! So it depends very much on the way you use OTRS.
