I have the exact same problem... but it isn't just okular -- it appears to
affect all KDE apps, even though I have confirmed that all settings point to
American English, en_US.UTF-8 etc. I've tried toggling them to British and
back... KDE apps always print a4, and it jams up the printer for everyone.
This has been a problem for years, and has followed me from distro to distro.
