Same story here with my Mandriva 2010:
Qt: 4.5.3
KDE: 4.3.2
I don't think it is a problem of Okular, as KWrite has the same problem (and
uses the same printing dialog). Printer asks for A4 paper, while page size is
set to Letter in print properties.
No problem with printing from OpenOffice or printing a test page.
