Aditya, I have identified the fact that the Chapter setup (most likely
the MPgraphic part of it?) is causing the footnote issue, nothing else.
But I happen to like this chapter setup for this particular book! Since
I cannot find what, in particular, about this setup causes the numbering
of footnotes to skip one when attached to the chapter title (I tried
adjusting various aspects of the setup, moving down line by line, but to
no avail), my only resource is to place the footnote immediately below,
where it is correctly numbered, then manipulate it in another program
later, so it can be placed where it needs to be.
Not the most adequate solution from a ConTeXt point of view I realise,
but since I am involved in production, with a publisher waiting and an
anxious author keeping a close eye on things, I have no other option at
this point, I feel.
Thanks anyway for alerting me to the likelihood that setups were the
likely problem.

It is quite likely that the setup that you posted below causes the error.
