All of the time?  Hmmm...  That's an interesting one.

A while back there was discussion of a macro, probably in vim rather
than in mutt, to scan for 'attach' in the message body and create the
relevant X-Attach: or X-Attachment: or whatever header; perhaps including
that header in your message will do it for you.

Lazur asked if anyone had such a thing, and I haven't seen any reply.
That might be enough to get you started, though; I vaguely recall
that creating the Attach: header by hand will attach a file (yep, just
tested it).
