Folder-hooks don't necessrily, but a "my_hdr From" will.

Right. A "my_hdr From" will override everything.

Yes, it's intentional. If you want to specify your From address
differently than what the Mutt default is, but still want to take
advantage of $reverse_name, then the method to use is to set the
$from variable to the address you want. $reverse_name will override
the $from setting, if applicable.
