Well, that depends how much flexibility you need.  If you always know
that the attachement you want to save is the 2nd part of the email,
and you have the email in a separate folder, you could create the
necessary Mutt commands for saving that on the command line with the
-e switch.  Ie. "view message components, go down 1, save, <name>,
enter, exit view, exit Mutt".  It should be possible to do something
like that so it doesn't require any interaction.

However, if you need any variable reference there, ie. you don't know
which component it is, or it's not the only/first message in a folder,
or things like that, then likely you can't do it with Mutt.
