I can reproduce this bug with

bash-4.2.45-1.fc18.x86_64
mutt-1.5.21-15.fc18.x86_64
ncurses-5.9-7.20121017.fc18.x86_64
glibc-2.16-30.fc18.x86_64

in xterm on Fedora 18.

After initial fg, I get rendered mutt screen back, but bash prints prompt and
processes input then. Second fg redraws mutt again, the shell prompt does not
appear again, input gets to the mutt, but mutt does not handle some keys
properly (it echoes them and does not trigger bound acation).

I will try Gentoo later to find possible differences and to debug it.
