With format=flowed, generating a message containing the lines
one blank
 two blanks
  three blanks
(like this message) each of the above lines is displayed with one
less space than is indicated by the numbers.  The actual message in
the mailbox does contain the correct number of spaces.

(In latest repository version, but also occurs with 1.5.13).

I am not familiar with the details of the space-stuffing conventions
for format=flowed, but since mutt both generated and is displaying
the message, it seems that mutt should either be adding an extra
space when creating the message (which it would then swallow when
displaying it), or should avoid chomping the beginning space when
displaying the message.
