If I compare this Visual Studio problem with trying to debug using
MonoDevelop, in MonoDevelop, the "Select Device" list shows all AVD images
greyed out until one is running, then I can select it.  The MonoDevelop
"Deploy To Device" output then shows:

> Detecting package list location
> Getting package list from device
> Installing shared runtime package on device
>       pkg: /data/local/tmp/Mono.Android.DebugRuntime-debug.apk
> Success
> 259 KB/s (28078095 bytes in 105.604s)

> Success

>

> Success
> 83 KB/s (71874 bytes in 0.844s)
>

> 100.00% complete
> Upload complete
> 

Then the MonoDevelop "Application Output" shows:

Then I can test my application using MonoDevelop.
