Did You try
adb kill-server
adb start-server
sequence?
this usually helps.
And yes check whether You can press on start button in MD iti is light
greyish so it confuses me too (sometimes)

If I start the emulator from AVD Manager and then hit F5 on
MonoDevelop, it
opens up the "Select Device" the same way: all off and "unavailable".

Check the emulator running, the AVD Manager with the emulator name
and the
MonoDevelop with the dialog window with emulator and "ok" button
turned off.
How do I tell MonoDevelop that the emulator is already up and running?
