The attached report (MAGIC) will find up to 15 dx codes and 15 proc codes
plus multiple doctors... This should do it for you.

I am trying to pull all of a patient's diagnosis' into a report and not
getting anywhere - does someone have this written already?
The report is for the Maryland Quality Indicator Project and I got the
