Rip, here's three different ones I've played with. The third one's the prettiest, I think. Note that for turning it into an on-demand report, you'd want to alter the selections on page 2 accordingly.
You could also just use one of these in-stream as the normal requisition for your RAD requisition, and just have it output the same information twice, if it's OK that the second copy just comes out together on the same printer.
