This is an ongoing issue...  

Tools we have found useful:
In OE, using the location lookup at the patient prompt  - L + space + unit 
mnemonic + F9. This works really well, and is pretty simple to remember the key 
strokes.
Using the Status board in combination with My list of patients today, or the 
Location lookup. Patients with the same last name are yellow to stand out. We 
set up nurses and CNAs first, before we could finish the rollout, we had set up 
a version for the secretaries and rolling that out too. 

Our standard is to use the account number, that is how they are trained...  
Lab, Cardio-Pulmonary etc get the same talk during training.  

We do counsel, not sure anyone has been fired (outside of registration.. where 
staff have been terminated)

Some of Brian's and Bill's vision for the status board would help as well.

We have a recurring problem with staff (physicians, nurses, 
ward clerks, etc.) placing orders on the wrong inpatients.  
Sometimes this is due to patients having the same or similar names.
Although staff are encouraged to identify patients by account
number they tend to identify patients by name.  This is not
a frequently occurring problem but one wrong order is too many.
Has your facility experienced this type of problem and have you
implemented any creative solutions?
