When we have a blood transfusion order for a patient we do the type and cross, order the blood and charge for the blood product on that date of service. According to our lab department they must place the charge for the blood on this account due to regulatory record keeping rules. The patient returns on a later date for the actual transfusion. Therefore the charge for the blood and for the transfusion service are on 2 different dates of service, 2 separate accounts, 2 separate insurance claims. This creates a problem when the patient is Medicare as Medicare requires the charge for the blood to be on the claim with the charge for the transfusion. Has anyone else had this problem and how have you resolved it?
