I agree that forwarding and distribution lists are the most likely reasons.

Another common possibility is what exim calls local_part_prefix and
local_part_suffix, often adding a "+" and a tag to the local part of
the address, but I imagine that your operators would recognise those.

I personally am another edge-case. I have a vanity domain where I have
access to all messages to the domain, and subscribed to many lists as
   me+list@mydomain
My hosting provider changed and I can now only send as me@mydomain,
so if I try to unsubscribe from these lists I would be a rarer
instance of this problem. I see that as my problem.

Undoubtably. That's why you should always use a token to identify
which subscriber to remove, not relying on the from address of the
unsub request.

Could they be forwards or distribution lists?

Happy Friday everyone and a Happy Passover/Easter/Good Friday to
all those that apply :) We?ve been using List-Unsubscribe with
both the ?mailto? and the ?unsubscribe url? for quite some time.
We have a back-end process that sends all unsub requests to a
mailbox in which we parse and opt subscribers out appropriately
according to each of our customers. We?re noticing that a handful
of unsub requests that come in don?t match up to the email
addresses that the messages were necessarily sent to. My guess is
that the devices that are being used to send the unsub mail are
using the the default email address of their mail client which may
not be the address that the email was sent to. The behavior
doesn?t seem to be ISP or domain specific as I?ve seen Yahoo,
Gmail, AOL, etc.. Does anyone have experience with this, that may
be able to make a suggestion as to how this works and if there is
anything that can be done?
