Hello David, There are only two heading levels by default, but you can add as many as you like. Off the top of my head, the option to customise the paragraph style is in the same contextual menu as the hotkey.
If you can't find it, I'll give you sensible instructions in a few days when I get back from training a teacher to use VoiceOver.
