Maybe I'm just lucky.  I never even once have experienced this.

At least not even near in the contexts you all describe. I've had it where sometimes I'll route my mouse to an object, then vo+shift+space to click, and it winds up clicking the apple menu instead, but that's about the closest thing even remotely that I've seen.
