The way this has ben done in the past involves the following. First all users enter Hangouts with Extras.. Visually impaired users use Chrome and install the Accesible Hangouts plugin made by Mohammed Mansuer, I believe. There are some tools that allow you record the Hangout, though I never did that part.
It is possible, to pull off, but does requires practice. There was even an attempt to create a call center using blind individuals as the operators, Google Hangouts as the interface, and some other pieces.
