According to this thread QuickTime X can't play MIDI files but QT7 can and so folks are encouraged to get that and install it. Actually the poster said QTX prompted him to get QT7 when they tried to play some midi files.

Midi playback actually works the same way it used to, but unfortunately, the 
quality is absolutely horrible now, so it would
almost be better if it didn't work at all. Until it gets fixed, your best bet 
might be to download something like vlc media
player and use it in conjunction with a sound font. To me, it doesn't sound 
nearly as good as midi playback under snow
leopard did, which is 1 of the many reasons I haven't fully switched over to 
Lion yet,  but it's a viable short-term solution
until apple gets around to fixing whatever they broke. Good luck, and I hope
