I'm not sure about heart attacks or stroke, but it will notify you if your 
resting heart rate goes above 120 BPM, you can do an ECG, and depending on 
which Apple Watch you have you could check your blood oxygen level. You can 
also use it to track your sleep. It isn't always accurate, but it should get 
better at it over time.

I understand the watch will dial 911 about 30 seconds after a fall
unless you interrupt that process. It will advise emergency services
that you have fallen and advise your location.
I don't know whether there's similar for heart attacks, stroke, etc.
Would be interesting to learn.
The medical support people I've talked with recently see the watch as a
worthy substitute for the older medic alert systems people would wear
around the neck. I find all this quite persuasive personally.
