Hi Zoe, your iPhone can only sync with one computer at a time, so you'd want to 
be sure you will be happy sticking with iTunes on the Mac before you make the 
jump.
If you do, the folder structure of your iTunes library is the same, whether you 
are using Windows or Mac. So if you have an external drive large enough to 
accommodate it, or your two computers are networked, you can copy the iTunes 
folder and all the folders underneath it across to your Mac. That will bring 
your music, apps, video and library data across. Then sync with your Mac.
It's been a while since I've tried this, and I don't recall if preferences are 
carried over as well. Perhaps someone else can comment on that. Since the 
folder structures are the same, it's actually a straightforward process.
