Hi! I have seen the first one, although I only get it when I change speech rate and voices occasionally.
As for Safari and editing text, VoiceOver on my end doesn't actually do that, although keyboard focus clicks are heard each time I type and move between lines in the same field, which is a weird behavior. That seems a little excessive. Most menus that Ive seen on websites I can get out of just by using VO-right, and VoiceOver then leaves the menu alone. That includes YouTube and Verizon Wireless for me, however, VOiceOver does excessively announce the menu rapidly for some reason.
