Sure. You open the app you want to set specific settings for. You go into
the Voiceover Utility, and there is a category near the bottom of the table
called Activities. All the options to do what you're asking about are in
there. Add a category, set your desired options, then set it to the app
which you just opened.

This then brings up another question, Can you have different Voice
over settings per app, a person uses?
Meanning lets say you want medium verbosity for text edit and nthe
lower verbosity for another app can this be done and if so how?
