If you access the iCloud Drive with cmd-shift-i, you’ll only see folders related to the Mac Apps that use your iCloud Drive such as Pages, Keynote, TextEdit etc and those folders that you’ve created yourself. Third party apps like VoiceStream Reader from iOS and others will not be visible either by VO or to the sighted user. This includes the photo library from Photos. If you go into this location with Terminal, the folders can be seen and interacted with using the “ls” and other commands, but they cannot be made visible using the typical Terminal commands to do this. So, they are present both in the Cloud and on your local machine, you just aren’t able to see them using regular Finder methods.

Yes. In the iCloud System Pref, in the Services Group, when you interact with the Services table, navigate to the iCloud Drive item, interact with it and press on the Options button. In the resulting dialog, you can enable/disable specific spa from using the iCloud Drive. Those that are disabled on your machine will not show up in your iCloud Drive for that machine. Also, note that there are quite a number of invisible items within that folder such as your photo library from the new Photos app and most third party folders like Voice Stream Reader etc.

To get to the iCloud Drive, press cmd-shift-i when in the Finder, or from any Open or Save dialog. There’s some dedicated folders in there that you shouldn’t remove related to the Apple suite of apps. You can add to or remove from these folders, just don’t remove them from this location except through the iCloud System Pref.
