Yes, restart your phone. Whenever you see that "could not load driver" message, 
that's the thing that seems to help. Your report is very odd, since 0000 has 
always been the pin for doing this (I don't doubt you, I just can't figure out 
why that would happen). You might want to go to Settings > Bluetooth, find the 
Apex, tap More Info, and tap Forget Device. After that, go back to VoiceOver's 
braille settings and try the pairing again.
