Nic, I interact with the table of content, and when I reach the  
chapter heading I want to read, I perform the click with vo+shift 
+spacebar and then stop interacting and vo+left arrow once and I  
usually am at the top of the page where my chapter begins. The only  
thing I noticed then is sometimes VO won't read the entire content of  
the chapter, or at least I suspect that sometimes it stops before the  
end. I also noticed that in when I try to read in this way and try to  
interact with the text in order to read slowly line by line, by  
interacting with the text I seem to be thrown to the beginning of the  
entire book as you describe.
