I hope this discussion can continue on the list because I have similar questions. I keep hearing sighted people rave about PDF Pen but I thought that was still inaccessible to VO users. I looked around AppleVis for PDF Pen references and could not find a thing.
When I used to use Word for Windows to build forms it wasn't too bad. I had to show the forms tool bar and there were buttons for each form widget. I just made sure to populate the help tags on all elements I put in the form and when the document was protected, you could tab from field to field and the help text would be automatically spoken by my screen reader. I was using Window-Eyes at the time. I'm wondering how accessible that process would be with newer versions of Office and especially on iOS or the Mac when that comes out. Problem with my suggestion so far is without a good working version of Word on a Mac, a Mac user wouldn't be able to use your form. I also wonder what would happen if you exported a form created in Word as I previously described into PDF format. But going back full circle, I don't think we have an accessible way to fill PDF forms on the Mac yet. This is an interesting subject indeed.
