Hi, Are you using the trackpad commander? It can occasionally be very easy to switch to the dock via that unintentionally if you have the default command set.
Also, do you have any applications docked or hovering around the top of the bottom of the screen (vo+f1 twice quickly should show them). Finally,check the position of the dock . Some times when I am in safari and using VO to go down a page out of nowhere instead of reading me what I am on on the page I start being read items that are in my dock but the dock isn't opened. If I VO to the right it will read me a line from the page then the next dock item. It isn't happening all the time, but enough that it is quite annoying. I can cycle VO off and on and it will stop for a bit but then just starts happening again, and I just rebooted my machine and it just started occurring again.
