You are definitely running the latest version of Disk Maker X?  It is, I 
believe, 3.0.4.  Anything prior to that will not give you VoiceOver 
accessibility.  

   If you are using the terminal method of doing this, be sure the thumb drive 
is blank, but formatted properly.  Also, I believe it has to be called, 
untitled, for the process to work successfully.
