Hi, and welcome,
We would like to help you, but the information provided is very limited.
It's not really your fault: LyX doesn't show any constructive error
message here, other than `it did not work.'
So we'll need more information. I assume that you have rtf2latex2e
installed and configured on your system. I would suggest that either you
a) Try running rtf2latex2e 'Avoidable-Losses-VIII-new-stuff.rtf' from a
command-line terminal, capture the output, and provide it here in email,
or
b) Send a .rtf this way, and hope that some kind person might be able to
convert it on their machine and post the result here.
