I don't know if it can help, but I tried your file on my LyX 1.5.4 on 
Windows XP and checked DVI, PDF and PS output and it is nearly OK.
Only a very little difference in the lengths in the PDF output. But 
nothing like your DVI ...
I didn't change anything in the file.
