Paul,

I take it that logo.gif and logo.pdf were written in the same directory
where the EPS
file lives
Yes.

You've probably already done this, but if not, I would suggest opening
figure_equity_vol_smile.eps in your EPS viewer of
choice (such as Ghostview, if you have that installed) just to see if it
displays properly.
My default viewer is GSview 4.8 and the .eps image displays correctly in it.

file lock
I am using WinXP Home so I can not view the security tab (which I believe is
an XP Pro feature). In any case, I have rebooted and am still having the
same problem.

I am not sure what to do at this point. I am thinking about reinstalling LyX
completely and not doing any MikTek updates. Does that sound like a
reasonable course of action?

That's good; it shows all the software is working. I take it that

I wonder if there is some sort of permissions problem with the EPS file,
or whether an application has a file lock on it. You can right-click
the file and go to Properties->Security to see the permissions.
Checking for a file lock is trickier, although you can be pretty sure
that a reboot will eliminate any locks on the file.

If it's not a permissions problem and not a file lock, the only other
thing I can think of is a problem with the content of the EPS file.
Since it works fine for me, I find that unlikely (but then, the entire
scenario is unlikely ...).
