It's hard to know without more complete information. But it sounds like it's an issue that arises from the OS's failure to save the file rather than a LyX problem. Are you saving documents to folders in which you have write permissions? Do you have adequate free space on your hard drive for swap files in addition to all your files? (OS X seems to like at least 1GB free disk space; I've found I run into trouble with less than 512MB free disk space.)

You might try checking to see if there's any output from Console.app when this problem occurs.
