I take it that \index{Rich Sheppard|see{Sheppard, Rich}} is not what you want, 
since this is described in man makeindex as well as in the makeindex 
documentation and in The LaTeX Companion 2.
IIRC "See also" (i.e. page number + crossref) is not possible with makeindex. 
xindy (a successor of makeindex) has something like this, but I think it does 
not work (yet) with LaTeX.

Read The Companion. It also describes xindy, which is much more powerful than 
makeindex, and, as of the latest release, even usable.
