I found a paragraph in the "Extended Features" doc on making an index and
I've scanned 'man makeindex'. Both of these describe marking words in the
document for inclusion in the index and setting nesting levels and other
physical layout/formatting issues.

  However, I find nothing on how to add cross references to the index ("See
also ...") or references to concepts that are not explicitly found in the
text body ("See ..."). We've all tried to find something in a book's index
but our word for the concept is different from that of the indexer so we
either don't find it or we need to hunt sequentially in the index to see if
it's there.

  I've never before compiled an index but I will need to do so for my book
as soon as I get the text to the publisher. That's at the end of this month.
So I'm trying to get a head start on the indexing process by learning how to
add to what LyX/LaTeX does automagically for me.

  BTW, any suggestions, guidelines or offers to help by those with
experience in indexing technical manuscripts will be greatly welcomed.
