I believe klyx could do this. The TOC dialog stuff is easy, but I have
no idea how the alteration of the document itself was actually done. But
I would certainly encourage you to have a go if you're interested :)

Long terms, the TOC dialog outlliner is not what we want, but it would
be a nice feature in the meantime
