Steve, Not applicable. But, about 15 years ago I used PSTricks to create a scannable form. Took time but worked. These 'forms' are not like a voting ballot. One is a checklist of compliance with the state's environmental policy act, another describes the project's compliance with the local critical areas protection needs. And so on. Question and answer type forms. Then there's the biological evaluation/assessment which requires addressing specific issues. Because I no longer have a typewriter, use windoze, or want to write manually I recreate the 'forms' in LyX.
