You're sure that there is no other xforms around? I've not seen that problem
with lyx-1.0.4. What does ``ldd /usr/bin/lyx'' report?

There's no guarantee that upgrading will fix that problem. I don't see
the crash happen with lyx-1.0.4 (I created a bunch of documents with embedded
EPS figures using lyx-1.0.4 without a single crash).
You could try upgrading if you want, since lyx-1.1.1 fixes many other problems.
However, I believe you have some other problem related to your system
configuration.
You could always wait a day or two and upgrade to lyx-1.1.2, as well.
