Why do we need 3 states for ERT insets (open, collapsed, and inline) ? In my opinion, the open state has no advantage over the inline state. So in my opinion, we can just have inline & collapsed states. Then, the ERT dialog can be removed (pressing the 2nd mouse button in an inline inset will collapse the inset, and pressing the 1st on 2nd mouse button on a collapse inset will make it inline).
