They said that it happened after (but not immediately after) copy/pasting a selection from a Word document into LyX that contained a picture. LyX asked to save the picture (as expected), the user saved the picture, and then 10 minutes or so later is when the dialog appeared. I'll ask them to keep an eye out in case it happens again.
