no problems at all. The index looks nice.
The manual of texindy reports an option for a log file: -t / --log-file
log.ilg

Is it possible to create this log file with Lyx in the Temp directory?
Best solution is, to add this log to Document > Latex Log (> Index).

This is off-topic to you problem but worth to be mentioned:
texindy is new in MiKTeX since August this year. So it was not available
for Windows when LyX 2.1.4 came out. In the meantime I added support for
this with a new installer. So just download the latest win installer for
2.1.54 you find at lyx.org and install it over your existing 2.1.4.
Please report back if you encounter problems.
