I think the 90% figure for modifying index entries is too high -- more like 50% in my experience. Nonetheless, I agree with Uwe that automatically opening the dialog is beneficial. On Mac, at least, were the dialog to open unnecessarily, closing it would be a single keystroke, but in cases in which the dialog needs to be opened manually, I have to use the mouse, taking my hands off the keyboard. So the inconvenience -- if it doesn't open automatically -- of having to open the dialog when I need it is far greater than the inconvenience -- if it does open automatically -- of closing it when I don't.
