What _I_ would like to see is a choice for the user of the output
encoding. LyX would use iconv to get this output. When one cannot find
a given unicode character in the output format, we would use a table
to convert. A starting point could be an extended version of the data
in 
http://www.tex.ac.uk/tex-archive/macros/latex/base/utf8ienc.dtx

(look for the \DeclareUnicodeCharacter declarations)

This table could then be used also by tex2lyx to transform TeX char
names to proper unicode codepoints.
