Could it be related to the place where your mouse cursor is (relative
to the place what the note dialog would have been if it was open)? 

What focus plicy do you use? Click-to-focus or focus-follows-mouse?

Thanks for the info. Unfortunatley, the lack of debug info for xforms
itself is clearly a problem. I just sent a message to
[EMAIL PROTECTED], the xforms maintainer, to ask whether he could send
you the xforms sources for debugging purpose.
