Use cdrdao (1.1.5), it uses cdparanoia to read the cd.

        cdrdao read-cd --device 0,1,0 cd.toc
Reads cd and creates two files, data.bin and cd.toc.

        cdrdao write --device 0,0,0 cd.toc
Writes data.bin to the CD using cd.toc as a layout file.
